These forms help us get to know you better and ensure a productive first session. Your initial in-person or online appointment will involve discussing your concerns, goals, and creating a plan tailored to your needs.

SELF-PAY:
I accept personal checks, debit/HSA cards, cash, and major credit cards including MasterCard, Discover, American Express, and Visa.

Teen Therapy: $165 (50-minute session)

Individual Therapy: $165 (50-minute session)

Couples Therapy: $185(50-minute session)

Parent Consultation: $165 (50-minute session)

INSURANCE:
We currently accept Medicaid insurance.

For all other insurance plans, we are an out-of-network provider. Medical receipts and/or super bills are available on the Patient Portal and may be submitted for reimbursement through your plan’s out-of-network mental health benefits, medical spending, or healthcare savings accounts.

For convenience and accessibility, we offer virtual appointments via SimplePractice, a HIPAA-compliant platform designed to keep your information secure. Whether you prefer in-person or virtual sessions, you’ll receive the same high-quality care.

We kindly ask for at least 24 hours’ notice if you need to cancel or reschedule your appointment. No-shows and cancellations made with less than 24 hours’ notice will incur the full session fee.

Our practice is conveniently located at 7955 E. Arapahoe Ct. Suite 1400, Centennial, CO 80122, in an ADA building with free off-street parking.
